As health and care in England shifts from hospital to community, there is growing demand for technologies that support neighbourhood working and integrated models of care.
Understanding how to navigate this evolving market is essential for companies who are developing or scaling high-potential products aimed at enabling the shift to neighbourhood health.
This edition of the DigitalHealth.London Pathfinder is focused on Neighbourhood Health and will support 20 companies within a focus on four priority areas:
- Proactive population health management
- Remote monitoring, outpatient transformation and virtual wards
- Interoperability and integrating healthcare information
- Service access and care navigation
Through this 10 week hybrid programme, companies will gain a deeper understanding of neighbourhood health priorities and practical considerations for commercial success.
This includes targeted support on procurement and commissioning; evidence generation; regulation; investment readiness; and commercialisation, helping companies with the knowledge, connections and market insights needed to bring innovative solutions into health and care systems and support more proactive, preventative and personalised care closer to home.
